At present Acute Healthcare is working with general practitioners, in line with practice based commissioning, examining and reviewing referrals into secondary care to assess the outcome of the referral and explore primary care services which could deliver care, in the community, working in line with the Department of Health to deliver healthcare closer to home. This work stream also encompasses reviewing patient pathways, accessibility and working in partnership with the practitioners and practice teams, to ensure skill mix, efficient and effective processes which are sustainable, in line with the Lean Thinking methodology, and maximising the use of resources.
These projects require our expertise to facilitate change management, service re-design and modernisation at a local level. Integral to this is clinical engagement at both a primary and secondary care level, to ensure care is delivered by the appropriate clinician, in the best setting, to enable patient satisfaction and ensure efficient capacity to meet demand in line with national Department of Health targets, for example the 18 Week Wait Target.
